SWEP provides a subsidy towards the cost of equipment and/or modifications to eligible participants. This is to improve independence in your home, assist in community participation and support families and carers in their role.

You must be a permanent Victorian resident, living independently and have a long-term disability and/or frail-age. For more information please refer to A&EP Guidelines.
How Do I Order Products?

All orders for mobility aids and equipment will be submitted to SWEP for assessment and approval.
Once the order has been approved and HospEquip receive the approval paperwork from SWEP we will contact you to arrange delivery.
